Text Box: GENERAL AUTHORIZATION FORM Joliet Diocese General Permission I request that my child _______________________________be allowed to participate in Harvest Sunday Date/Time November 2, 2003 12:00-2:00 PM and November 9 12:00-5:00 PM. I am aware that my child will be driven by an adult over the age of 21 for this event. I release and indemnify St. Joseph Parish, Bradley, IL. Its staff, volunteers, and the Joliet Diocese, from any and all liability arising from claims of any kind or nature whatsoever from my child participation in this event. Videotaping and Still Photographs Videos and still photos will be taken during this event. Event registration constitutes permission for possible participation in the video or still photos. These may be used for future promotional efforts. Code of Behavior (All Participants) You are representing Youth Ministry in the Joliet Diocese and your parish during this event. We expect that you display mature and responsible behavior, which for many years has been the trademark of Catholic youth and adults of our diocese and parish. Here are some of the expectations for behavior: All participants are to arrive on time. All participants are expected to show common courtesy and respect at all times. Inappropriate language or behavior will not be tolerated. Socializing should always be done in public areas. Attire/ swimwear should reflect the value of modesty and Christian values. The possession or consumption of any alcoholic beverages, and /or possession/ use of any illegal drug by an individual is not permitted. Smoking is not permitted. Weapons and/ or drug paraphernalia are not allowed. If under the age of 18, prescription drugs need to be given to an adult from your parish for storage and distribution. Infraction of these rules can mean immediate dismissal with no refund. Participants will be responsible to local authorities as well. I understand and agree to this behavior code. I also understand and agree that at the time of an infraction requiring my dismissal, I am responsible for my removal from the premises, and any costs involved. If under the age of 18, I also understand and agree that my parents or guardian will be notified at the time of an infraction requiring my dismissal.
